Avera McKennan Hospital Worker Type: Regular Work Shift: Day Shift (United States of America) Position Highlights The clinic pharmacist serves as an integral member of the interdisciplinary care team, providing comprehensive medication management to clinic patients. This role includes assessing, initiating, monitoring, and adjusting pharmacotherapy for acute and chronic conditions, both independently and while working directly alongside medical residents in co-visits to support clinical decision-making and enhance resident education. The pharmacist functions as a key drug information resource for providers and staff, ensures evidence-based care, and contributes to safe, effective medication use across the clinic.
Additional responsibilities include the management of patients with substance use disorders (including medication assisted treatment), support of maternity care through medication optimization in pregnancy and postpartum, and longitudinal management of chronic diseases. This position emphasizes team-based care, population health outcomes, and improved access to high-quality patient-centered pharmacotherapy services.
