Work Shift: 8 Hours - Day Shifts (United States of America) Scheduled Weekly Hours: 40 Compensation: Salary Range: $34.00 - $51.00 $15,000 Sign on Bonus Union Position: No Department Details RN Navigators at Coordinated Treatment Center collaborate closely with multidisciplinary teams across a diverse range of patient populations and diagnoses to coordinate comprehensive, patient-centered care. Day-to-day

Responsibilities

  • - rooming patients, obtaining vitals, weights, and other clinical measures - supporting team members to ensure patients are seen by the appropriate disciplines - monitoring patient progress to confirm orders and education are completed - addressing ongoing patient needs through phone, MyChart, and other follow-up communication methods Role Includes: - Mon-Friday schedule - $15,000 Sign on Bonus - Competitive

  • Ability to facilitate groups, multidisciplinary teams, and implement best practices when necessary.
