Avera at Home Milbank Worker Type: Regular Work Shift: Primarily days with night, weekend and holiday on-call involvement (United States of America) Position Highlights You Belong at Avera Be part of a multidisciplinary team built with compassion and the goal of Moving Health Forward for you and our patients. Work where you matter. A Brief Overview Accountable for ensuring patient care is coordinated and managed appropriately.

Focuses on the work of the department, quality monitoring, teaching and serving as an ongoing resource. What you will do Demonstrates ability to plan and be responsible for the day to day operations of the assigned agency. Receives case referrals and reviews available patient information related to case, to determine home care needs.

Assigns appropriate personnel to case as needed. Attends care conference meetings with hospice/home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. Oversees the implementation and evaluation of in-service and continuing education programs for staff.
