Summary
- The RN Patient Flow Coordinator is responsible for optimizing patient throughput and ensuring efficient patient movement throughout the facility. This role coordinates admissions, transfers, and discharges while maintaining high standards of patient care, safety, and satisfaction. The RN Patient Flow Coordinator collaborates closely with nursing units, physicians, case management, and ancillary departments to balance patient needs with available resources and capacity. Under the leadership of the Nursing Director, works with multiple disciplinary teams to optimize patient throughput. Day to day operations of patient flow and sustainment of our early discharge process. Provides operational leadership for assigned duties which includes hands-on unit presence and direction in facilitating provision of patient care and achieving organizational outcomes. Responsibilities
- Patient Flow Management Coordinate and prioritize patient admissions, transfers, and discharges across all units.
- Monitor real-time bed availability, staffing levels, and unit capacity to facilitate optimal patient placement.
- Serve as the central communication hub for bed management and patient movement.
- Collaborate with Emergency Department, perioperative services, and inpatient units to reduce wait times and bottlenecks.
- Clinical Coordination Review patient acuity and ensure appropriate level-of-care placement.
- Utilize clinical judgment to assign patients based on medical needs, safety considerations, and organizational protocols.
- Support continuity of care by ensuring timely transitions between departments.
- Collaboration & Communication Communicate effectively with physicians, nursing leadership, case management, and support services.
- Facilitate interdisciplinary discussions to resolve patient throughput challenges.
- Escalate capacity concerns and coordinate surge planning as needed.
- Operational Efficiency Track and analyze patient flow metrics to identify trends and opportunities for improvement.
- Assist in implementing strategies to improve throughput, reduce length of stay, and enhance patient experience.
- Ensure compliance with hospital policies, regulatory requirements, and patient placement guidelines.
Qualifications
- Education: Associates Degree Nursing required. Nursing Licensure
- Current State of New Mexico or Compact State Nursing License required Certifications: BLS required Specialty Certifications in area of practice and/or management preferred. Experience: 2 years experience as a clinical nurse. Skills

Must have an understanding of human resource issues, operational budget, and financial resources.
