Summary
- As a Home Health Hospice RN, you will provide compassionate, patient-centered care to patients with life-limiting illnesses in their home setting. Using the nursing process, you will assess, plan, implement, and evaluate individualized care focused on comfort, symptom management, and quality of life. This role requires effective coordination with patients, families, caregivers, and the interdisciplinary care team to ensure timely, holistic, and supportive end-of-life care while utilizing resources efficiently and adhering to evidence-based standards. Responsibilities
- Serves as a clinical resource by coordinating patient care and delegating responsibilities based on patient needs and staff competency.
- Provides direct hospice nursing care , evaluates outcomes, and adjusts care plans to promote comfort, dignity, and quality of life. Collaborates with patients, families, physicians, interdisciplinary team members, and community resources to ensure coordinated end-of-life care. Educates and supports patients and families regarding disease progression, symptom management, advance directives, and care planning.
- Maintains knowledge of hospice best practices, clinical standards, regulatory requirements, and relevant clinical equipment.
- Communicates effectively with the care team to support collaboration, timely intervention, and quality patient care. Participates in on-call responsibilities, including phone triage and home visits as needed.
- Maintains competency in pain management, symptom control, medication management, wound care, and palliative care interventions. Supervises home health aides in compliance with regulatory and organizational standards. Performs other duties as assigned.
Qualifications
- Degree from an accredited nursing program required (programs under conditional status may be approved by System CNO).
- Minimum Associate Degree in Nursing (ADN) required.
- Current New Mexico RN license or Compact State RN license required.
- Basic Life Support (BLS) required for all RN roles (AHA, ASHI, or American Red Cross accepted).
