Job Title: Registered Nurse Coordinator Cost Center: Womens and Children Job Description: The RN Coordinator/Specialty Program Coordinator is responsible for the coordination of care and ongoing management of the patients in the respective specialty program and/or department. Assumes leadership role to facilitate interdisciplinary collaboration in caring for and meeting the needs of the patients in the respective specialty program and/or department. Functions as a clinical liaison and resource for the respective specialty program and/or department. Minimum Qualifications ◾ Education Graduate of an accredited school of registered nursing. ◾ Credentials Must possess a current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN license for current state of practice. BLS certification is required. Additional certifications preferred: completion of CPI Work Environment A patient care setting. Exposure to blood and body fluids, contagious disease, hazardous chemicals, dangerous fumes, medical gases, and exposure to out of control patients, which may result in physical and/or verbal abuse. Physical Demands Able to stand, walk, bend, squat, reach, and stretch frequently. Possess physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs. Needs adequate hearing and visual acuity, including adequate color vision. Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects. Able to use proper body mechanics to assist patients in ambulating, transferring in and out of bed, chair or wheelchair. May be required to lift up to 75 pounds. Must use standard precautions due to threat of exposure to blood and bodily fluids. Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. May require periodic use of personal computer. Essential Functions Assumes responsibility for the coordination of care and ongoing management of the patients in the respective specialty program and/or department. Functions as the organization's clinical liaison and resource for the respective specialty program and/or department. Assumes responsibility for assisting the organization in achieving and maintaining certifications and accreditations specific to the respective specialty program and/or department. Activities may include but are not limited to participating in site visits and tours of the facility, performing random chart audits, and rectifying any identified issues. As appropriate and pertinent to the specialty program and/or department, rounds on patients and assesses and monitors continuity of care. Expedites and coordinates delivery of services to facilitate a patient's progress throughout the continuum of care. Facilitates patient, family, and caregiver education to promote continuity of care after discharge as well as optimal patient outcomes. Reviews and revises patient education materials. As appropriate performs telephone follow-up surveys on patients post discharge. Collects and maintains statistical and other data that is required to be collected and maintained by both the organization and/or others for the respective specialty program and/or department. Participates in PI (Process Improvement) and quality improvement programs and projects that serve to improve the quality and outcomes of the services and care provided to patients in the respective specialty program and/or department. Process improvement and quality improvement programs and projects may also be required to maintain certifications and accreditations of the respective specialty program and/or department. Collaborates with and as appropriate takes direction and carries out orders from physicians, specialists, and other providers for the delivery of care and services to the patients of the assigned specialty program and/or department. Participates in community outreach and education efforts to promote and support the assigned specialty program and/or department. As necessary and required by the organization and other parties leads and organizes these outreach and education efforts. Other duties and responsibilities as assigned with heightened emphasis to those that pertain to enhancing the patient experience through direct patient, family and care giver contact. Join us.
