Schedule

  • Monday – Friday • Practice includes 4 Family Medicine Physicians and 2 Physician Assistants. • Provide comprehensive primary care, including examinations, diagnostics, DOT physicals and treatment. • Analyze findings of tests and examinations, and diagnose conditions of patients. • Determine and prescribe medication, dosage, and schedule given the patients' conditions and allergies. • Instruct patient and families regarding medications and treatments. • Refer patients to specialists when necessary. • Educate patients about their health conditions, medications, and treatment plans. • New resident/fellows are encouraged to apply.   • Job Description Family Medicine Core Privileges includes the ability to perform history and physical examination, admit (if applicable to the setting), evaluate, diagnose, treat and provide consultation to patients of all ages with a wide variety of illness, diseases, injuries, mental health conditions and functional disorders of circulatory, respiratory, endocrine, metabolic, musculoskeletal, hematopoietic, gastroenteric, integumentary, nervous, female reproductive, and genitourinary systems. This includes the pre-operative care of surgical patients and post-operative medical care of surgical patients and return to work issues.
  • Qualifications M.D. or D.O. degree. Experience can be demonstrated in one of the following ways and should be reflective of the privileges being requested: An applicant who has just completed residency shall provide their residency procedure log and a list of completed rotations, as applicable. OR An applicant who is not applying directly out of a residency shall provide a quality profile from hospital(s) where he/she currently has privileges showing his/her clinical activity for the last 12 months, including numbers of procedures performed morbidity, mortality, infection rates, and other complications. Applicant must be able to demonstrate active involvement in the clinical aspects of Family Medicine by providing case log that includes 25 vaginal deliveries, 50 adult medical patients, and 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable to privileges being requested, in the last 12 months. Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or he Royal College of Physician and Surgeons of Canada. Training at residency programs accredited by organizations other than the above, may be considered via the waiver of eligibility process as defined in the Credentialing Policy. Certification in the primary area of practice by the appropriate specialty/ subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA), as applicable. Those applicants who are not board certified at the time of application but who have completed their residency training within the last five years shall be eligible, however must achieve board certification within five years from the date of completion of their residency training unless the appropriate governing Board waives such requirement.
