Summary
The CNS or NP serves as a clinical resource and an independent practitioner who diagnoses and manages acute and chronic diseases in the area of advanced practice licensure. Proactively manages the care of patients across the continuum of care. Oversees the care management of complex patients.
Job Description: Practice Highlights: 14-16 patients per day Wound Treatment Assistants available per provider Chronic wound & ostomy management: evaluation and assessment, diagnosis, treatment, including debridement of wounds and application of appropriate dressings, establish plan of care and maintain continuous treatment No call obligation Daily
Schedule
Monday-Friday 7:30am-4:00pm Student Loan Forgiveness eligible under PSLF and NM Health Professional Loan Repayment Program Job Requirements *Compiles and analyzes data from multiple sources to independently formulate differential diagnosis, plan of medical management/plan of care, and monitors and evaluates outcomes for the patients served. *Accurately orders and interprets appropriate diagnostic tests. Prescribes necessary treatments and/or medications. Performs minor surgical procedures.
Provides referrals to other disciplines for services identified to meet patient's needs. *Performs health maintenance and health restoration activities that meet the emotional, physical and medical needs of the clients served. Manage acute and chronic diseases. *Work autonomously as well as in collaboration with other members of the health care team when identified issues are beyond the APRN scope of practice. Delegates appropriately. *Coordinates the plan of care with the patient & family and across all related disciplines to facilitate quality patient care and optimal outcomes.
Participates in interdisciplinary care conferences. Serves as patient advocate. *Supports superior outcomes by participating in job related projects which may include research, data collection, quality assurance, policy and procedures/standards of care, teaching activities, and program development to improve the quality of patient care and continually evaluate our effectiveness. *Serves as a role model to promote excellence in practice by consistently utilizing evidenced based practice principles. Serves as a resource to other providers/disciplines *Performs other functions as required Additional Job Description: Masters degree in Nursing.
