Summary

Responsibilities
- Adheres to the LPN scope of practice Administer prescribed medications and treatments, as ordered by provider/Standard of Care Protocol.
- Provides patient care in the ambulatory care setting according to established standards in collaboration with Providers, Registered Nurses and Medical Assistants as members of the care team.
- Provides primary support to the provider by proactively anticipating the provider needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures. Participates on interdisciplinary team to meet patient care and clinic needs, working cooperatively with others to accomplish required work in a timely and efficient manner. *Assists providers with procedures. Assists with patient education, within scope of practice by Standard of Care-Education Protocol. Assists with visit planning, within Health Maintenance Guidelines. Assist with management, in collaboration with Clinical Manager. Documents in electronic medical record within scope of practice. Assists in screening of patients , in collaboration with Care Manager. Assists Certified Medical Assistants with patient preparation/rooming patients, as directed by visit plan. Applies Compassionate Connected Care principles and incorporates into care interactions Interacts effectively with customers, providing instructions and information, both written and verbal, as appropriate.
- Maintains established practice policies and procedures, objectives, quality program, safety, environment, and infection control standards. Seeks help, communicates, and cooperates with Medical Provider and team appropriately. Meets assigned productivity targets Performs other functions as required.
Qualifications
