Job Title: Clinic LPN GPPN Cost Center: Urgent Care Job Description: The Clinic LPN is responsible to check patients in upon their arrival. Using EMR the nurse will obtain vital signs, Review of Systems; medication lists, family and social histories, and reviews the medical records for completeness before the physician sees the patient. This person makes sure appropriate supplies are available for the patient's visit and any procedures as well as makes sure that the rooms are sterile and safe. Additionally the nurse will triage patient phone calls, medications requests, support physician with in office procedures and administer logs such as refrigeration control and sample medication receiving/distribution. Minimum Qualifications o Must possess a current, valid LPN or RN license in state of practice, temporary LPN or RN license in state of practice, or compact LPN or RN licensure for current state of practice. o BLS certification is required. Additional certification or continuing education may be required based on area of practice. Physical Demands o Able to stand, walk, bend, squat, reach, and stretch frequently. o Possess physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs. o Needs adequate hearing and visual acuity, including adequate color vision. o Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects. o Able to use proper body mechanics to assist patients in ambulating, transferring in and out of bed, chair or wheelchair. o May be required to lift up to 75 pounds. o Must use standard precautions due to threat of exposure to blood and bodily fluids. o Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. o May require periodic use of personal computer. Essential Functions 1. Ensures quality patient care and adheres to provider preferences for their patients. Explains procedures, treatments and reinforces provider recommendations to patients using the AIDET process. 2. Provides prescribed sample medications in accordance with approved nursing techniques. Administers injections/immunizations as prescribed by providers. 3. Uses EHR to record visit, medication(s) and entry of patient information such as vital signs, ROS, medication lists, family and social history accurately as well as provides point of care testing. Records test results, reports and other procedural documentation in patient chart and reports to providers for further instructions or sign off. 4. Maintains sample medications by logging their receipt. Sample medications must be documented in the patient's chart. Sends written, verbal or computer generated requests for prescription refills to pharmacies as requested by provider. 5. Tracks lab and procedure orders. Communicates clinical data test/procedure results accurately to the healthcare team. 6. Initiates a patient education plan as prescribed by physician or clinic policy. 7. Communicates and instructs plan of care to patient/family/guardians. Documents understanding of instructions given in the patient's chart. 8. Completes and documents patient phone calls in the patient's chart. 9. Assists with office procedures as requested by physician. 10. Administers IV medication as prescribed by physician.
