Registered Nurse (RN) Case Manager – Remote – Oncology experience required Work from home from anywhere in the U.S.! We are seeking a compassionate and clinically skilled RN Case Manager to join our team and help members navigate their healthcare journey through comprehensive case management and care coordination. What You'll Do Conduct telephonic and occasional face-to-face assessments to evaluate member healthcare needs.

Develop individualized care plans that improve health outcomes and overall wellness. Coordinate services, resources, and programs to support members with complex medical and social needs. Apply clinical expertise to identify risks, remove barriers to care, and facilitate successful transitions across the healthcare continuum.
