20 hours/weekWork hours generally between 8am and 5pm, Monday-Friday
Shifts negotiable, 4-hour days @ 5 days/week, or two 8-hour days and one 4-hour day @ 2.5 days/week
Onsite position at MMC Minocqua on Hwy 70
Occasional travel or weekend/evening hours, would offset the daytime hours (community outreach events across the service area)
Summary
As a trusted liaison between the Marshfield Clinic Health System, the VA, and the community, the Veteran Service Care Coordinator facilitates access to essential services for Veterans. The role involves screening for medical and social needs, mental health conditions, and VA eligibility to optimize care coordination and clinical outcomes. Functioning as a key member of the healthcare team, this individual provides clinical, clerical, and technical support to bridge gaps in Veteran care.
Job Description
Veteran Care Coordinator connects Veterans with healthcare services, community resources, and the VA to improve health outcomes and ensure smooth care across systems. The role includes assessing Veteran needs, offering social support and coaching, and developing individualized action plans covering military history, VA benefits and eligibility, preventive health needs, and personal health goals. A strong understanding of Veteran benefits is essential for helping Veterans access earned services through the VA and community partners.
Coordinators collaborate closely with healthcare teams—including clinicians, discharge planners, and community partners—to support safe and effective transitions of care. This includes helping Veterans navigate post discharge needs and ensuring continuity between hospital, VA, and community-based services.
Administrative responsibilities include verifying service authorizations, maintaining communication with the VA, tracking program data, and ensuring compliance with Sanford Health and VA standards. The role also contributes to system and process improvement by sharing best practices, developing educational materials, and assisting with staff and clinician education.
Overall, the coordinator supports program goals focused on seamless VA–community care integration, addressing unmet needs (especially for rural Veterans), and strengthening Veteran-centered services through collaboration, advocacy, and a deep understanding of the Veteran experience.
Qualifications
Completion of the following courses or the military equivalents: Medical Terminology, Anatomy & Physiology or Body & function or Human Body in Health & Disease, and Pharmacology required.
Experience working or volunteering with direct customer service experience in human services, healthcare, community outreach, health education, or related direct service and proficiency with written and verbal communication skills and Microsoft Office Suite, one year required, two preferred.
Experience working with the needs of the Veteran population or Military service preferred.
Preferred: Current AAMA or RMA certification. CHW Certification. Motivational Interviewing (MI) Certification, Certified Health Coach (CHC) through the National Society of Health Coaches (NSHC); Health Coach Certification through the American Council on Exercise (ACE); Health and Wellness Coach Certification through Well Coaches School of Coaching; or Coach Training Certification through Coaches Training Institute (CTI).
Graduate of an LPN program, or documentation of nursing education and training through an accredited Registered Nurse program that meets state requirements to take LPN boards.
Completion of military training: Medical Specialist (68W), Hospital Corpsman (HM), Aerospace Medical Service Specialist (4N0X1), Combat Medic (68W), Independent Duty Medical Technician(4NOX1C), Pharmacy Specialist (68Q), Pharmacy Technician (HM-8404,4T0X1).
