Home Connections – Home Care Aide JOB SUMMARY Join our team and be a part of our mission to deliver accessible, high-quality, affordable, and compassionate healthcare. Prairie Lakes Healthcare System is a non-profit healthcare system serving 10 counties in northeastern South Dakota and west-central Minnesota. Our team at Prairie Lakes makes a difference in the lives of patients and their loved ones POSITION SUMMARY Full-Time(0.8 status or 32hrs per week) Wage: $16.25-$24.76 Under the supervision of an RN, PT, or ST, provides assistance with activities of daily living, personal cares, homemaker skills, and delegated exercises. Wage: RESPONSIBILITIES Follows the delegated plan of care to meet the needs of the patient in the home setting. This may include personal cares, housekeeping tasks, and delegated exercises. Observe and report physical and/or behavioral changes in the client to the Home Connections office, Home Health/Hospice nurse, or Department of Human Services case worker. Documents patient/client care provided in a timely and accurate manner. Assists in maintaining a clean and safe patient/client environment. Follows the assigned schedule and informs clients if the scheduled appointment to the patient/client's home needs to be modified. Completes annual aide competency training and participates in the ongoing aide training. Provides the Home Connections office with an accurate and timely record of mileage and other required employee-centered documentation. JOB SPECIFICATIONS Education and/or Experience Graduation from high school or in possession of a GED certificate. Completed Nurse Aide Certification program or has completed or is eligible for completion of homecare aide qualification training. Maintains knowledge and skills by attending required hours of in-service training. Previous experience desired. Must have or be eligible for South Dakota's Drivers License.
