Clinical Case Manager

The Clinical Case Manager enrolls and facilitates medical care for eligible newly diagnosed HIV individuals in the Basin Assistance Services service area within 72 hours of diagnosis. The position fulfills the case management duties of the Rapid StART clinic with the goal of having available access to care that aligns with education provided to clients addressing importance of medical adherence and retention in care, ultimately leading to a lower community viral load and decrease the amount of new diagnosis. This position works independently, under limited supervision, reporting major activities through periodic meetings.
