Care Coordinator

The Care Coordinator works closely with the behavioral health patient, doctor and other clinic staff to ensure the patient has the resources and tools necessary to improve their mental health by providing access to high quality physical and behavioral healthcare in order to help reach wellness and recovery of the whole person. The Care Coordinator works closely with other team members to perform screenings, assessments and intake documentation in order to successfully enroll patients into services. This position is also responsible for closing the gap in treatment by linking patients to access multiple types of services within the community and provide follow up to ensure the patient receives services with those providers to achieve their recovery goals. This position works independently, under limited supervision, reporting major activities through periodic meetings.
