Position Details

The Mental Health Case Manager provides case management for individuals with severe mental illness, coordinates services and advocates for the patient. This position assists patients in accessing medical, social, educational and other services to improve quality of life. Service coordination includes crisis prevention and management, monitoring of services, service planning and coordination. This position works independently, under limited supervision, reporting major activities through periodic meetings.
