Job Description

The ED Care Coordinator is responsible for effective communication with physicians, nurses, and all ancillary departments to ensure continuity of patient care and maintain efficient flow of the department. The ED Care Coordinator is also responsible for effectively communicating with outside facilities (hospitals, transportation agencies, physicians, etc.) to coordinate transfer of patients from the ED for tertiary care or higher level of care facilities.

SHIFT AND SCHEDULE

Full Time, Days, 7:00am-7:00pm

ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS

  • Answer multiple phone lines and transfer calls appropriately while exhibiting professional phone etiquette.
  • Answer patient call lights as appropriate and communicate patient needs with appropriate staff members in a timely manner.
  • Assist with plan of care coordination to include obtaining patient information, completing documents, and documentation in electronic health record to ensure timely throughout.
  • Demonstrates proficiency in the ability to navigate the electronic healthcare record (EHR) and other various electronic programs to obtain patient information and provide timely, accurate documentation.
  • Provide effective communication with charge nurses, physicians, mid-levels and other team members both in and out of the department. Completes accurate documentation regarding the patient’s clinical condition as needed to secure post-acute services or services requiring a higher level of care. Appropriately arranges transport necessary for continuum of patient care.
  • Receive incoming faxes and ensure documentation is entered into the appropriate EHR.
  • Tracking information to follow current and appropriate admission process to include regularly checking hospital email for most up-to-date processes.
  • Utilize the hospital paging system appropriately as needed for patient care.
  • Respects patient’s rights and maintains confidential information.

EDUCATION AND EXPERIENCE

  • High school diploma or equivalent.
  • Associate’s degree preferred.
  • Prior medical background experience preferred.

  • Stand, walk, sit, stoop, reach, lift, see, speak and hear. Lifting is limited to 35 lbs. for clinical staff and to 50 lbs. for non-clinical staff. The individual must use an assisted-lift device or get another individual(s) to assist with the lift that is over these maximum limits.
