Disease Management Nurse - Remote

## Job Description

Join Sharecare as a Disease Management Nurse working remotely to provide telephonic case management and health coaching services. You will work with patients to develop personalized care plans, coordinate services, and support optimal health outcomes through evidence-based interventions.

**Time Type:** Full-time (40 hours per week)

## Essential Functions

- Conduct telephonic assessments and develop individualized care plans for patients with chronic conditions
- Provide health coaching and education to patients and families
- Coordinate care services and communicate with healthcare providers
- Monitor patient progress and adjust care plans as needed
- Document patient interactions and maintain accurate records
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Identify and address barriers to patient care and health management
- Support patients in navigating healthcare systems and accessing resources

## Qualifications

### Required
- Bachelor's degree in Nursing (BSN) preferred, or Associate degree in Nursing (ADN) with relevant experience
- Current RN license in good standing
- 3+ years of clinical nursing experience, preferably in case management, chronic care, or disease management
- Strong assessment and critical thinking skills
- Excellent verbal and written communication abilities
- Proficiency with electronic health records and computer systems
- Ability to work independently in a remote environment

### Preferred
- Certification in case management (CCM) or chronic care (CDMS)
- Experience with telephonic or remote patient care
- Knowledge of chronic disease management principles
- Bilingual capabilities (Spanish preferred)
- Experience with Sharecare or similar health technology platforms
