As a Community Resource Specialist (CRS) at One Health , you’ll be a frontline advocate for patients, connecting them with vital resources and support systems. This patient-facing role is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing. • Coordinate Care: Facilitate warm hand-offs and referrals between patients, providers, and community services.
• Screen & Support: Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
