Position Highlights
Position Summary:
The Registered Nurse (RN) Heart Failure (HF) Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. This RN HF Coordinator will be working with cardiology patients who have a diagnosis of heart failure. The goals of care include long term management of HF to improve the patient’s quality of life so they may remain active members of their families/society and promote self-management of a chronic disease state, promoting autonomy. Titration of medications for HF patients will be in collaboration with the HF Team, which includes a Pharmacist and a Provider, thru the use of order sets and protocols. The RN is responsible for independently implementing and carrying out the plan of care established by the physician either through delegation to the appropriate departments or completing the plan themselves.
Work Schedule:
Monday through Friday with variable work hours
Education/Experience:
Graduate of a nursing program. Two years of critical care experience is preferred
Licensure/Certification:
Current licensure of registered nursing in the states of South Dakota, Iowa, and Minnesota as required. Current BLS and ACLS certification. Heart Failure Certification thru the AAHFN-CB required within 6 months of accepting the position
