A Brief Overview
Serves as a patient liaison to help patients and family members "navigate" the complex healthcare systems. This position will work with a multidisciplinary team providing improved comprehensive behavioral health services to patients. This position requires expertise to identify and implement improvement processes, and the ability to design, direct and implement teaching plans. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers. The ultimate goal of the program is to provide care that is coordinated and navigated along an agreed care path with all information available to patients and families as needed, and thus, minimizing barriers along the full continuum of care.
What you will do
- Serves as a patient and family advocate for patients and families at Behavioral Health by coordinating patient care services and eliminating operational barriers to services, assuring excellence in patient care and patient flow. This includes providing support to navigate the healthcare system and being available to patients and families throughout their care as an open, knowledgeable and empathetic contact, providing support during the inpatient process to explain outpatient services.
- Develops, organizes and participates in educational activities for patients and their family. This includes providing support to navigate the healthcare system and being available to patients and families throughout their care as an open, knowledgeable and empathetic contact, providing support during the inpatient process to explain outpatient services.
- Collaborates with other health care providers in developing, implementing, and evaluating educational materials for patients and their families including family education sessions on the unit.
- Communicates with referral providers within patients are admitted and discharged from behavioral health inpatient hospital.
- Assures that appropriate referrals are made for patients.
- Conducts follow-up conversations, discharge phone calls, as needed with all patients and/or families and communicate concerns, changes, or social needs in patient health to appropriate MD or other appropriate care providers.
- Provides education through formalized routine groups or classes to meet identified unmet needs in the community.
- Establishes and maintains positive working relationships with key internal and external customers such as primary care physicians, outpatient therapists including developing patient referral services by relationship-building with physicians not related to the department including assisting with counselor symposiums and/or Friday Forums.
- Provides follow up calls and case management to patients and families.
- Leads group therapy and education sessions for patients and families impacted by behavioral health.
Essential Qualifications
The individual must be able to work the hours specified. Required Education, License/Certification, or Work Experience:
- Bachelor's in human services filed such as social work, psychology, behavioral sciences, or nursing
- 1-3 years Mental Health Experience
Preferred Education, License/Certification, or Work Experience:
- Community Health Worker
