Join Our Team as a Behavioral Health Care Manager! Are you passionate about making a difference in the lives of others? One Health is looking for a dedicated Behavioral Health Care Manager to join our integrated care team.

Position Summary
As a Behavioral Health Care Manager, you will be a core member of an integrated behavioral health team, working closely with patients' primary care providers in clinic and/or school-based settings. Your role involves coordinating referrals, supporting behavioral health services, and ensuring adherence to state and federal confidentiality requirements. You will work under the direction of licensed behavioral health and medical providers to help patients access the care they need.
Employment Type: Full-Time (40 Hours per week, Monday- Friday 8am-5pm) Salary (Wage): $18.68-$19.89/hr DOE Location: Hardin Campus Key
Responsibilities
- Coordinate: Support and coordinate behavioral health services with the patient’s integrated care team.
- Engage : Connect with patients during warm hand-offs to offer support, education, and resources.
- Facilitate : Assist patients in accessing services using approved multi-media platforms and ensure follow-up care.
- Track: Monitor patient follow-up and clinical outcomes, documenting encounters in the EHR.
- Refer: Facilitate referrals for clinically indicated services specified on the patient’s treatment plan.
Qualifications
- Required: High school diploma or equivalent, valid driver’s license, and competent computer skills, including Microsoft Office.
- Preferred: Bachelor’s or Associate’s degree in a related field, experience in a medical clinic or mental health setting.
